Lower Your Risk for Diabetes


According to the Illinois Department of Public Health, 1.5 million new cases of diabetes are diagnosed each year. In Illinois, 12% of adults have diabetes, over 3% of the national rates. You may be at risk of developing Type 2 diabetes if you have prediabetes, are overweight, have a family history of diabetes, poor diet, and being physically active less than three times a week.

Loyola Medicine's FREE National Diabetes Prevention Program, which you can sign up for below, offers a yearlong lifestyle change program aimed to prevent or delay the onset of type 2 diabetes. Participants will learn skills to eat healthy, increase physical activity, and manage stress. The program meets weekly for 16 weeks, then bi-weekly, and then monthly for the remainder of the time. Designed as a lifestyle modification program, this program promotes modest weight loss (5-7%) and increased physical activity through a 12 month lifestyle change program.

This national program is designed to support individuals in making healthy lifestyle changes to improve their health. Learn how to be more active, manage stress and lose weight while learning techniques to help you stay positive and motivated. The Diabetes Prevention Program includes 16 weeks of classes followed by monthly maintenance sessions to provide ongoing support for optimal outcomes.

The Diabetes Prevention Program occurs over a year and participants are required to:

  • Attend weekly sessions – consisting of 16 sessions
  • Attend maintenance sessions – one or two times a month throughout the year
  • Self–monitor weight
  • Track and learn more about healthy eating and physical activity
  • Learn how to problem-solve and stay motivated

Our specially trained lifestyle coach will support you in:

  • Becoming more active
  • Working through negative thoughts
  • Losing a moderate amount of weight
  • Controlling your portions
  • And more!

Participants will learn to:

  • Identify and change eating habits
  • Add physical activity to your life
  • Manage stress
  • Stay on track when eating out
  • Understand and respond to your food cues
  • Stay motivated
  • Solve problems that can get in the way of healthy changes.



 

Eligibility


The Diabetes Prevention Program is open to adults who meet the following criteria:

  • Are at least 18 years old and not pregnant at the time of enrollment
  • Are overweight (body mass index equal to or greater than 25; equal to or greater than 23 if Asian) and
  • Have no previous diagnosis of Type 1 or Type 2 diabetes

AND

Meet one of the following:

  • Have a blood test result in the prediabetes range within the past year
    • Hemoglobin A1C: 5.7%–6.4% or
    • Fasting plasma glucose: 100–125 mg/dL or
    • Two-hour plasma glucose (after a 75 gm glucose load): 140–199 mg/dL OR
  • History of gestational diabetes mellitus
