UPDATE: Power outages caused by storms have been resolved at Palos Health South Campus, Elmhurst and River Forest outpatient clinics. Palos Health South Campus has resumed appointments as scheduled. Immediate Care at River Forest will reopen today, August 12, 2020, beginning at noon. Appointments at the Elmhurst clinic will resume as scheduled on Friday, August 14.

Contract Participation

Trinity Health Accountable Care Organization (ACO)

Loyola University Health System is a member of Trinity Health, a national Catholic healthcare system. Loyola Physician Partners is one of four Trinity Health organizations participating in the Center for Medicare & Medicaid Innovation's (CMMI) ACO model called the Next Generation ACO (NGACO).This model advances the administration’s healthcare system goals and builds on experience from the Pioneer ACO Model and the Medicare Shared Savings Program. Through this model, CMS partners with Trinity Health and other ACOs that are experienced in coordinating care for populations of patients and whose provider groups are ready to assume higher levels of financial risk and reward. Other Value-based Contracts

Loyola Physician Partners currently participates in a number of Commercial, Medicare Advantage and Medicaid contracts with varying levels of risk and reward. Providers may be eligible for quality incentives or shared savings depending on network performance on cost, utilization and/or quality metrics.

For all of LPP's arrangements, the health system has made the commitment that physicians will not be at risk if we do not reach our performance goals. We continuously seek to improve our performance, strengthen our network, and share tools and information needed for success. Physician engagement is key to success in these arrangements. We know providers are busy juggling multiple priorities so we have created a succinct physician ASK that outlines physician expectations and what you can expect from us.
