House Calls Program
Loyola offers primary care house calls for homebound patients in parts of west suburban Cook County. This includes those with mobility issues or other medical conditions that make it difficult to leave home, such as dementia. The program currently accepts Medicaid patients and those with Medicare plus Medicaid.
Our team includes a physician that provides primary care services, as well as nursing, social work and pharmacist support to keep you or your loved one healthy at home. We offer management of chronic health conditions such as heart failure, diabetes, dementia, high blood pressure and COPD. Our team can help with obtaining medical equipment, giving immunizations, refilling medications and advance care planning to prepare for future decisions about your medical care.
Our patients gain access to a network of 15 other partners to address social vulnerabilities that lead to poor health outcomes such as senior issues and access to food, housing, and behavioral health care.
