Our therapy programs emphasize an individualized approach with specific interventions that are tailored to your specific needs. We focus on providing the highest level of clinical care as well as engaging you in your treatment plan. We encourage you to ask any questions you may have at any time about your treatment. Our goal is to help you return to all of your previous activities without pain or other symptoms and to provide you with the knowledge needed to prevent problems in the future.
Patients can be referred for services by a primary-care physician, specialist or advanced practice nurse. The results of your initial evaluation will be provided to your physician within the first week and subsequent status updates will be sent as needed.
Individual insurance plans vary, so we encourage you to check with your insurance group regarding the exact coverage and benefits that are authorized. As a courtesy, we will bill your insurance company monthly. Any remaining balance that is not covered by your insurance plan is your responsibility. If you have any questions regarding your bill, please feel free to ask any of our service representatives. If they are unable to assist you, they may direct you to the Patient Financial Services Department. For your convenience, their direct number is 708-216-5000.
Appointments can be made during our office hours either in person or by calling 888-LUHS-888 (888-584-7888). The initial evaluation is scheduled for one hour and subsequent appointments average 45 to 60 minutes.
We attempt to schedule your treatments with the same therapist throughout the course of your therapy to provide consistency in your program. However, if there is a conflict between your desirable appointment times and the therapist’s available time, we will work with you to find a therapist that meets your scheduling needs. We would appreciate 24 hours notice should you need to cancel or reschedule your appointment. This will allow us to accommodate other patients by making that time available. To receive the maximum benefit from therapy, it is critical that you attend scheduled appointment sessions. We reserve the right to discharge a patient from therapy after three occurrences of canceling or failing to show up for a scheduled appointment. A letter will be sent to your referring physician notifying them that you have been discharged from our program due to attendance issues.
We are happy to provide copies of your medical records. These can be obtained by completing an Authorization for Disclosure of Medical Records form obtained at the front desk. Our service representatives will forward your request onto our department to complete your request.
